Online submissions require an account registration and an unsworn declaration with date of birth (DOB will be redacted after submission).

Submit a Notarized Complaint

Notarized complaints must be filed using one of the forms available on the Commission's website and submitted by physical mail, hand delivery, or e-mail to sworncomplaints@ethics.state.tx.us. Please click here if you wish to submit a notarized complaint.
